Photo ID, your insurance card(s), and a list of your current medications.

Insurance we accept

We see patients with Aetna, Ambetter by Sunshine, CarePlus, Cigna, Florida Blue (PPO only), Humana, Medicare, Optimum, Sunshine State Health Plan, and UnitedHealthcare. We also accept plans that use these networks. Coverage varies by plan, so please call if you are unsure about yours.
